What to Expect
As a creditor under the Federal Trade Commission's "Red Flags Rule" we adhere to an Identity Theft Prevention Program. For this reason, we ask that you be able to present photo identification at the time of registration. We appreciate your cooperation in assisting us with protecting your identity and the prevention of identity theft.
Acceptable forms of photo identification include:
•  Valid State Identification Card
•  Valid State Drivers License
•  School Identification Card
•  Valid Passport
We ask you to arrive at least 15 minutes prior to your scheduled appointment time in order to provide sufficient time to complete all necessary paperwork.  
We are pleased to offer translator services at no cost to our patients. If a translator is needed, when scheduling the appointment, we ask that you notify us at this time.
Please wear comfortable clothing.
Please plan on staying for at least 1 hour or more for your assessment.
